You can use this helpful checklist to make sure you are staying on top of your responsibilities.


 

New Employee

Have new employee complete the enrolment form within in the waiting period to ensure employee will be covered from the effective date.  Medical Evidence forms are not required, unless eligibility for Life and/or LTD exceeds Non Evidence Maximums (NEM).

All employees and their dependents must be covered by their Provincial Healthcare Plan (eg. OHIP) before they can be covered by your Group Plan.  If provincial replacement coverage is required please contact us.

Distribute cards, booklets, etc. upon receipt, but not prior to effective date.

 

Termination of Employee

Notify the Insurance Company or our office when an employee terminates.     Advise employee of conversion option within 30 days of termination for the Life Insurance benefit.  If employee requires continuation of Health and Dental benefits, please have them call us within 60 days of termination to discuss.

 

Salary Changes

Notify the Insurance Company or our office promptly of any salary changes (as applicable for salary based coverage for Life, AD&D and Disability benefits).  NOTE – Employers may be held liable if employees suffer a loss of benefit at claim time if the Insurance Company has not been notified of change in earnings.

 

Excess Coverage (for Life and Disability)

Advise employees when they are eligible for excess coverage beyond the Non Evidence Maximum (NEM) and provide the Medical Evidence form to them.  Completed forms should be returned to you in a sealed envelope and then forwarded promptly to the Insurance Company.  If the employee does not wish to complete the Medical Evidence form, then a written statement to this effect should be obtained from the employee and placed in their personnel file for reference. 

 

Billing Statements

Thoroughly check monthly billing statements for accuracy to ensure they reflect additions, terminations and rate adjustments at renewal time.

 

Member Lifestyle Changes

Notify the Insurance Company or our office within 31 days when an employee undergoes a “lifestyle change”.     For example, marriage (including common-law), divorce, birth or adoption, name change, etc.  NOTE – Penalties may apply if this time frame is not observed.

 

Supplies

Ensure there is always a supply of booklets at hand.  Additional materials can be ordered directly from the Insurance Company or our office.
