| Lifestyle Changes |
Notify the Plan Administrator if you get married, divorced, move-in common-law, have a baby (including adoption) or experience a death in your family within 31 days of the event. Any subsequent changes to the family (eg. Additions, dependents no longer eligible for student status) should also be communicated to the Plan Administrator in a timely manner. Original forms indicating any beneficiary changes resulting from a lifestyle change must be submitted to the Plan Administrator in a timely manner. | |
| Annual Updates |
Provide the Plan Administrator with proof of dependent student status each fall (if applicable). | |
| Loss of Card or Spousal Coverage |
Notify the Plan Administrator immediately if your drug card is lost or stolen. Notify the Plan Administrator within 31 days if you lose spousal coverage. | |

Please contact your Plan Administrator directly for administrative inquiries. If you require assistance with claims, please contact the customer service centre at the Insurance Company. Please remember to keep copies of all correspondence. |
